Patients who are new to the Ahmanson/UCLA Adult Congenital Heart Disease Center should have their doctor’s mail or fax in cardiac related medical records prior to scheduling an appointment. Please include the following:

Cardiology Progress Notes
Echocardiogram Reports (including stress echo’s and transesophogeal echo’s)
Cardiac MRI/CT
Electrocardiograms (ECG)
Cardiac Catheterizations
Surgical reports
Recent Labs

In addition to the above records, we require a copy of the patient’s echocardiogram on CD or VHS tape for studies performed within a one-year period from the date of the appointment. This can usually be arranged with the organization that performed the study.
